Target audience: Physicians, nurses, social workers, medical students, psychologists, licensed counselors, dietitians, and other health professional students, educators, healthcare staff, front office people, schedulers, and others. Individuals who identify with the populations sharing their stories (veterans, people experiencing homelessness, and the transgender community) and community allies.
Learning Objectives:
At the completion of this activity, the participant should be able to:
1. identify key health care gaps affecting veterans, individuals experiencing homelessness, and transgender patients, and describe how social drivers of health contribute to inequitable outcomes.
2. describe structural factors that impact access to health care and health outcomes for patients with complex health and social needs, including veterans, individuals experiencing homelessness, and transgender patients.
3. identify communication techniques that mitigate mistrust among patients who have experienced discrimination or fragmented care systems.
4. interpret patient narratives with humility, including recognizing how stigma, trauma, and past negative encounters may shape patient engagement.
